Where is the “Burial Wishes” Act 4 lore book?

I've been through the Act many times and have not seen this drop. Where is it?

Answer

It's actually in the Act IV hub (the Barbarian keep), north of the waypoint. It's on the corpse of Martha.

Go there first thing in the Act. It is rather easy to miss actually.
