Where can I find these Act I quest lore books?

I'm missing two quest lore reports for an achievement: "Report from Wortham" (the first) and "Villager's Journal" (the last).

Where can I find them?

The villager's journal will pop on a dead body lying under a hanging tree during the Fallen Star quest in Act 1 on the old Tristram road.

I found the report on a dead body named Dead Villager from Wortham or something similar, he was very well marked. It was just prior to getting to Leoric's palace as @Alok mentioned, in the Northern Highlands.
