Help identifying PC game where you could wear enemies' heads

My memory is a bit fuzzy on this, but I will try my best.

Time frame is late 1990's, 1995-1996?

The game played out as a 3rd person shooter I believe, though it wasn't bloody or that violent. The main gameplay mechanic of the game was the fact that you could wear different heads or masks that gave you different powers (or guns, can't remember). You could simply shoot the head off of an enemy, and swap his head with the one you were currently using to change powers. There were 100+ different kinds in the game, according to the game's box.

I believe this game was bundled in past at a Sam's Club as a 3 pack with two other games. I think one of the games was JumpStart Adventures 3rd Grade: Mystery Mountain.

Unfortunately, I no longer am in possession of this game as it was returned to the store shortly after playing it once, but it's been bugging me that I can't remember what it was called.
